We’re here to give you the support you need, when you need it the most

We’ll help you to maintain or regain your independence if you’re struggling with your mental health.

Our wellbeing workers work closely with the NHS to support customers with poor mental health to continue living independently in their own home.

Providing a link between housing, health and other wellbeing services, our team will support you if you’re feeling anxious, depressed or are finding it hard to cope following an admission or discharge from hospital. We’ll help you to improve your wellbeing and support you in times of crisis when you need some extra help and support.

  • Following hospital discharge
  • To access essential services
  • To pay your rent and other bills.

External agencies can refer customers to the service by contacting us for a referral form.
